Using the Website, submitting a contact form, sending an email, or requesting an appointment does not create a provider-patient relationship. Such a relationship is formed only when you are seen and accepted as a patient by one of our licensed providers. Medical knowledge changes quickly, and we do not guarantee that Website content is complete, current, or applicable to your individual situation.

Our collection and use of information through the Website is described in our Privacy Policy. Protected health information that we create or receive as your healthcare provider is handled as required by HIPAA and applicable state law, as described in our Notice of Privacy Practices. If these Terms conflict with our Notice of Privacy Practices regarding your health information, the Notice of Privacy Practices controls.
